I understand and acknowledge that participation in a Texas Elite tryout is voluntary and that Texas Elite does not provide health, medical, accident, disability, or other personal insurance coverage for participants attending a tryout.
I understand that I am responsible for maintaining adequate personal health and/or accident insurance for myself and/or my child(ren) and that my personal insurance coverage will be the primary source of coverage for medical treatment, injury, illness, property damage, or other expenses arising from participation in or attendance at a Texas Elite tryout.
I accept financial responsibility for deductibles, copayments, coinsurance, uncovered medical expenses, and other costs not paid by my insurance.
By participating in a Texas Elite tryout, I acknowledge that I am relying upon my own insurance coverage and financial resources for such expenses and am not relying upon Texas Elite to provide insurance coverage for me or my child(ren).
