FINANCIAL RESPONSIBILITY AND PAYMENT POLICY I understand and acknowledge that participation on a Texas Elite team involves a financial commitment. I understand that I will receive a season statement reflecting the fees and other amounts associated with my player’s participation. Payment is due upon receipt of a statement or, for recurring monthly obligations, on the 1st day of each month. I understand that I am responsible for keeping my account current and in good standing throughout the season. I understand that failure to maintain a current account balance or comply with an approved payment arrangement may affect my player’s eligibility to participate in practices, games, tournaments, team activities, or remain on the active playing roster. Payments made to Texas Elite are non-refundable, except when otherwise expressly approved by Texas Elite or required by applicable law. I understand that withdrawing, resigning, being removed from a roster, missing practices or games, or otherwise choosing not to participate does not automatically entitle me to a refund or relieve me of financial obligations already incurred. By completing this registration, I acknowledge that I have read, understand, and agree to these financial responsibilities and payment terms. *

PERSONAL INSURANCE AND FINANCIAL RESPONSIBILITY I understand and acknowledge that participation in Texas Elite Fastpitch is voluntary. Texas Elite may maintain team, organizational, or other insurance coverage; however, such coverage is not a substitute for personal health, medical, or accident insurance and should not be relied upon by participants or their families as their primary source of coverage. I understand that I am responsible for maintaining adequate personal health and/or accident insurance for myself and/or my child(ren). My personal insurance coverage will be considered my primary source of coverage for medical treatment, injury, illness, and related expenses arising from participation in or attendance at Texas Elite practices, games, tournaments, training sessions, team activities, tryouts, or other events. I understand that the existence of any insurance policy maintained by Texas Elite does not guarantee coverage or payment of any particular claim, injury, loss, expense, deductible, copayment, coinsurance, or other cost. Coverage, if any, is subject to the terms, conditions, limitations, exclusions, deductibles, and eligibility requirements of the applicable policy. I accept financial responsibility for deductibles, copayments, coinsurance, uncovered medical expenses, and other costs not paid by my personal insurance or any other available coverage. By registering and permitting my child(ren) to participate with Texas Elite, I acknowledge that I am relying primarily upon my own insurance coverage and financial resources and not upon any insurance policy maintained by Texas Elite to provide personal insurance protection for me or my child(ren). *
